If you have pre-existing surrogates that you would like to add in to Proxy, simply use the Add A New Surrogate button

Then fill in Name, DOB and any of the other fields you wish and click Save.

Then you will have the opportunity to enter more comprehensive demographic information on the following page. When you have finished filling out the information, click Save.
