To add an Intended Parent or new patient to Proxy, click the chart icon/Intended Parents.

Click the Add a New Patient button:

Then fill in all of the fields below and click the Add Patient button when complete.

You will then land on the Patient Details page where you may enter further patient and partner information. When completed click Save Patient Information.

The IP/Patient information is now saved to Proxy and a chart has been created.
